Jacksonville – Samantha Ann Stevens "Sammy", 41, passed away unexpectedly on October 25, 2025, at her home. No services will be held at this time.

Sammy was the beloved daughter of Tom and Betty Stevens. She will be remembered for her sense of humor, laugh, and firey personality.

She is survived by her high school sweetheart and longtime partner, Donnie Smith; twin daughters, Madison and Mackenzie Smith; parents, Tom and Betty; and brother, Alex Stevens.
